Safety Policies
It is our intent to provide a safe and healthy environment, Sapphire Counseling and Assessments, LLC asks that you please follow the following policies listed; No dangerous weapons on the property. No use of threatening or intimidating behaviors. No possession of alcohol, illegal drugs, Kratom, Delta 8 or 9 on property. Medications must be within your possession at all times.
Payment Policy
Our services are billed by an affiliate billing company, Business management Solutions. Our providers participate in most all of the major health insurance plans in the region. Please be aware that with any health plan there may be deductibles and co-payments for which you could be responsible. In our office, payment is expected at the time of service.
Weather Policy
If Lincoln Public Schools are closed due to inclement weather, our office will be closed as well. Some providers may choose to still see clients through online telehealth sessions so please check with your provider.
Cancellation Policy
We ask that all clients kindly notify us of an appointment cancellation at least 24 hours in advance. If you are unable to give us 24 hours notice, your account will be billed $50. This fee is not covered by insurance and must be paid before your next appointment. This policy does not apply to Medicaid and Medicare clients. Thank you.
