Safety Policies
It is our intent to provide a safe and healthy environment, Sapphire Counseling and Assessments, LLC asks that you please follow the following policies listed; No dangerous weapons on the property. No use of threatening or intimidating behaviors. No possession of alcohol, illegal drugs, Kratom, Delta 8 or 9 on property. Medications must be within your possession at all times.
Payment Policy
Our services are billed directly through our office. Our providers participate in most of the major health insurance plans in the region. Please be aware that with any health plan there may be deductibles and copayments for which you could be responsible. Please check with your insurance company prior to scheduling your session. In our office, payment is expected at the time of service. A valid credit card is required on file for all clients, any balances over $250, we reserve the right to run your card on file.
Weather Policy
If Lincoln Public Schools are closed due to inclement weather, our office will be closed as well. Some providers may choose to still see clients through online telehealth sessions so please check with your provider.
Cancellation Policy
We ask that all clients kindly notify us of an appointment cancellation at least 24 hours in advance. If you are unable to give us 24 hours notice, your account will be billed $50. This fee is not covered by insurance and must be paid before your next appointment. This policy does not apply to Medicaid and Medicare clients. Thank you.
