Who we are...
We are a small outpatient practice of mental health therapists and substance use counselors who have been affected by addiction and/or mental health issues within our own families and want to help others.
Sapphire Counseling and Assessments, LLC was founded from a desire to "Do Better" in the community and to offer a non-judgmental and safe environment for all.

Shelley Baldassano
Therapist/Business Owner
Shelley is a Licensed Independent Mental Health Practitioner and a Licensed Alcohol and Drug Counselor.
She has over 16 years experience helping people who struggle with mental health related illness and addiction. Shelley also sits on the board of the Nebraska Chapter for NAADAC.

Business Manager/Owner/ RN
Justin is a Registered Nurse and business owner.
He enjoys helping others and has a passion for movies and horror films.
Justin will be helping us by scheduling appointments, answering phones and keeping the business organized.

Maureen Drake
Therapist/Business Associate/Supervisor
Maureen is a Licensed Independent Mental Health Practitioner and Licensed Alcohol and Drug Counselor.
She enjoys helping others and really enjoys the listening aspect of counseling.
Maureen offers mental health and substance use counseling. She also utilizes DBT skills in session. 402-429-5711

Safety Policies
It is our intent to provide a safe and healthy environment, Sapphire Counseling and Assessments, LLC asks that you please follow the policies listed here; No dangerous weapons on the property. No use of threatening or intimidating behaviors. No possession of alcohol, illegal drugs, Kratom, Delta 8 or 9 on property. Medications must be within your possession at all times. No ESA animals allowed on property, only appropriate service animals.
Payment Policy
Our services are billed in through our office. Our providers participate in most all of the major health insurance plans in the region. Please be aware that with any health plan there may be deductibles and co-payments for which you could be responsible. In our office, payment is expected at the time of service. A valid Credit card is required to be on file for all clients, any balances over $50, we reserve the right to run your card on file.
Weather Policy
If Lincoln Public Schools are closed due to inclement weather, our office will be closed as well. Some providers may choose to still see clients through online telehealth sessions so please check with your provider.
Cancellation Policy
We ask that all clients kindly notify us of an appointment cancellation at least 24 hours in advance. If you are unable to give us 24 hours notice, your account will be billed $50. This fee is not covered by insurance and must be paid before your next appointment. This policy does not apply to Medicaid clients. Thank you.

Psychiatric Diagnostic Evaluation: $350
Substance Use Assessment: $350
Psychotherapy, 60 mins: $250
Psychotherapy, 45 mins: $200
Psychotherapy; 30 mins: $150
Family psychotherapy, with patient; 50 mins: $200
Group Therapy; 50 mins: $200
Therapeutic activities, direct patient contact, 15 mins: $20
Telephone evaluation and management services, 5-10 mins: $12
Telephone evaluation and management services, 11-20 mins: $20
Telephone evaluation and management services, 21-30 mins: $30
Court Appearance, price per hour with travel: $300
Submitting records, reports, letters as needed, price per 20 min: $$35
Cancelations and "No Show" sessions within 24 hours; $50
(except Medicaid)
